Cardinal VBE was created in response to the U.S. Department of Health and Human Services (HHS) issuing Final Rules on December 2, 2020, allowing for Coordinated Care. The HHS Office of Inspector General and HHS Centers for Medicare and Medicaid Services both issued final rules for providing Coordinated Care. Coordinated Care must be provided under certain frameworks for compensation, but a safe harbor is provided.

Cardinal VBE assists Primary Care Practices, Surgeons and Medical Facilities to work in a coordinated manner. The goal of this coordinated care is to provide for better patient outcomes and reduced healthcare costs.
Cardinal VBE provides continuous patient care. After the primary care practice
closes the patients have an APP that provides telemedicine on a 24-7 basis. If the
patient needs to go to the emergency room. Cardinal VBE can help provide
coordinated emergency care by partnering with hospitals and primary care
practices.
For example, when a partnered primary care patient utilizes a partnered hospital
ED department, the hospital can have immediate access to that patient’s primary
care records. Additionally, Cardinal VBE will send out a transitional care plan to
the Primary Care provider when that patient is released from the hospital.
This process allows a typical primary care practice to provide better care and
reduce ER visits by 40% as it consults with the patient to determine the need for
ER care.


Cardinal VBE can help provide coordinated care with surgeons by monitoring the care between partnered primary care providers and partnered surgeons.
For example, when a partnered primary care provider sends a patient to a
partnered surgeon, the Cardinal VBE bridge software will send coordination
updates between the provider and surgeon.
Upon release from the medical facility, a coordinated transitional care plan for
both the surgeon and provider has resulted in lower readmittance rates.
The primary care provider will receive transitional care billings, and the surgeon will have an increased case load.
Cardinal VBE can help provide coordinated care with partnered practice patients
and the partnered medical facility by providing immediate patient medical history
to the ED department. Additionally, Cardinal VBE will notify the primary care
provider of the medical episode.
Upon release from the medical facility, a transitional care plan will be sent to the
primary care provider, and the medical facility will receive increased case load
from partnered primary care providers.
Utilizing partnered primary care practices, the medical facility should receive
enough ED visits to fully utilize the ED facilities.
Surgical cases from the ED visits and partnered primary care referrals should
fully utilize the OR facilities.
Cardinal Medical can provide well care for primary care providers and their patients.
For example, Cardinal Medical nurses and techs can provide annual wellness exams. Additionally, the medical staff can perform wellness ultrasound exams, ABI screenings, wellness screenings and cognitive tests.
Well Care Services can materially help with CMS MIPS scores to enhance payments up to 9% on CMS patients. Net payments for these services can increase practice revenues in excess of $500,000. We cover the costs of these services and the Primary Care Practices are not billed until fees are collected.

Patients can continue to receive care even after the practice closes. This can reduce
the risk to permanently losing patients to a system that has longer hours.
For example, Cardinal Medical nurses and techs through telecare and wireless devices
can provide patient care on a 24/7 basis. Additionally, Cardinal Medical can provide
chronic care management.
These Well Care Services can materially help with CMS MIPS scores to enhance
